COngestive Heart failure Extra pharmacotherapeutic Support Individualized On patient Needs (COHESION)
Description
Congestive Heart Failure (CHF) has a high impact on healthcare and society. Multidisciplinary multicomponent transitional interventions, including pharmacist care and tailored to the individual patient, have shown promising results. Therefore, this project aims to develop a longitudinal and personalized pharmaceutic care program for patients with CHF, to assess the effect of this program on guideline directed medical therapy (GDMT) and to determine factors relevant for sustainable implementation. The intervention will be co-created with patients and other relevant stakeholders including community pharmacy franchise organisations, building upon the components that have been shown to be important for quality of CHF care in literature. The starting point of the intervention is in the hospital, enabling to include a high risk population. After this starting point, all components of the intervention are aimed at keeping the patient outside the hospital with a good quality of life as long as possible. The first step in developing a longitudinal multidisciplinary pharmacotherapeutic support program for patients with CHF is a context analysis including interviews with patient and all relevant stakeholders. Building upon the results of the context analysis, the intervention will be developed using the Intervention Mapping model. The developed intervention will be tested in an evaluation study using type 2 hybrid effectiveness controlled design. Main outcome will be the proportion of patients adhering to GDMT. A health technology assessment (HTA), including a cost-effectiveness evaluation will be part of the project, as will the exploration of factors relevant for sustainable implementation.
