Medication Information Transfer and Medication Reconciliation
Description
This study examines the effectiveness of the National Medication Transfer Program using a pre- and post-test design. First, a literature review was conducted to differentiate between healthcare sectors with high and low risk of medication discrepancies between what a patient actually uses and what is listed in the system of healthcare providers.
In addition, observations were conducted in two hospitals to assess discrepancies in several high-risk sectors (e.g. hospital care, general practitioner care, community pharmacy care). This includes identifying medication discrepancies between medication lists across the high-risk sectors, time measurements of medication reconciliation.
In low-risk sectors (e.g. outpatient mental healthcare, disability care, and thrombotic services) questionnaires were used. Medication discrepancies and time investment in medication reconciliation were estimated by healthcare providers.
Additionally, patients' and healthcare providers' experiences were surveyed to assess experiences with medication information transfer via questionnaires.
The results of the studies are used to develop quality indicators, which aim to continuously evaluate the program's impact on the quality and efficiency of care.
