Hospital Pharmacy Days – November 28 & 29, 2024
During the Hospital Pharmacy Days, a series of insightful presentations were delivered by researchers and PhD students:
- Kjell van der Walle presented a seminar on prescribing cascades, guiding the audience through the background and strategies for addressing them. The session included an interactive explanation of prescribing cascades, presenting various studies from our research group, tools and practical recommendations for healthcare providers to identify, address, and prevent prescribing cascades in clinical practice.
- Afke van de Plas demonstrated that risk-based double-checking of parenteral medications can not only enhance patient safety but also save nurses valuable time.
- Judith de Ruijter-van Dalem presented findings showing that medications initiated in the hospital are often continued more than six months after discharge. This raised the question: How realistic is it for patients to still require medications, such as sleeping medication, after such a prolonged period?
- Ruveyda Yilmaz (ANIOS OLVG), nominated for her research, showed that prescribing cascades are more frequent at higher dosages of certain medications.
- Charlotte Falke (Lecturer and PhD candidate, Division of Pharmacoepidemiology and Clinical Pharmacology, Utrecht University) stressed the need for a more comprehensive focus on the complexity of a patient's overall pharmacotherapy during medication reviews.
- Jetske Graafsma (PhD, UMCG) provided an engaging presentation on the efficiency of medication monitoring, showing that while progress has been made, there is still much work ahead.
Yannick Nielen, Valmira Isufi, and Lieke van Dinter served on the program committee, contributing to the success of the event.
