Prisma symposium

A large delegation of our department Clinical Pharmacy and Toxicology attended the PRISMA Symposium on 12 May 2026. The symposium provided an excellent opportunity to present research rooted in daily pharmaceutical practice and to engage in discussions on how pharmacotherapeutic care can be further improved.
Several studies focusing on medication safety, adherence, and innovative pharmaceutical care were presented, including:

  • Veerle van Hulten: Data-driven detection, management and prevention of prescribing cascades.
  • Betsie Limmen: Prevalence and impact of medication discrepancies across care transitions: A prospective observational cohort study in Dutch hospitals.
  • Sabeth Spronk: Secondary data analysis of Adverse Drug Reactions reported by healthcare professionals and patients: interventions to manage ADRs.
  • Valmira Isufi: Verbeterde herkenning en aanpak van voorschrijfcascades: een gecontroleerde evaluatie van de CASC-ADE tool).
  • Emma Tijmensen-Reijers - Stichting Apotheek der Haarlemse Ziekenhuizen (SAHZ): Electronic transmission of discharge medication reports to community pharmacies and post-discharge dispensing discrepancies: a before–after study. 
  • Fatma Karapinar: ChatGPT-5.2 vergeleken met ChatGPT-4o voor het samenvatten van bijwerkingen.
  • Charlotte Falke - Universiteit Utrecht: Medicatie gebruikscomplexiteit en zorggebruik bij de oudere patiënt na ziekenhuisontslag.
  • Lieke Roeke: Real-world medication adherence and medication reviews in adults and children with cystic fibrosis.
